A Nursing and Midwifery Council fitness to practise hearing into the conduct of a former Hywel Dda University Health Board funding assessor is due to conclude today (Friday).

Lisa Jane Callen, worked as a band 6 NHS funding assessor for the health board. She has is appeared in front of a fitness to practise hearing this week charged, with failing to accurately assess patients.

The Nursing and Midwifery Council (NMC) hearing was due to examine 25 charges, relating to the assessment of seven service users between December 2012 and January 2013.

In the case of all seven, Ms Callen is charged with failing to accurately assess and / or record and/or complete a nursing needs assessment, and of copying information from earlier assessments carried out by colleagues.

In the case of five of the service users it is alleged that Ms Callen did not complete accurate assessments on more than one occasion.

Ms Callen is also charged with failing to recommend a falls review in one case and failing to accurately assess and / or record and/or complete a nursing needs assessment in relation to elimination or continence in another.

In the case of the final service user it is alleged that she did not raise the fact that the service user appeared to be administered medication covertly and/or record the appropriate staff responses.

She is also charged with failing to recommend that the same service user be seen by dietician or undergo a speech and language therapy assessment or review.

The NMC charges state that in all nine instances of copying her colleagues’ earlier reports Ms Callen’s conduct was dishonest.

“You had sought to demonstrate that you had assessed the relevant service users’ needs and/or completed an appropriate/ accurate nursing needs assessment in those areas when you had not,” reads the final charge.

“In the light of the above, your fitness to practise is impaired by reason of your misconduct.”

The hearing is due to conclude this Friday, November 27.
